Rib Cartilage Rhinoplasty

When the nose lacks support, cartilage grafts may be needed to build a new framework. If septal cartilage is insufficient, ear or rib cartilage is considered.

Rib cartilage is used mainly in noses that have had previous surgery with septal cartilage already used, in significant collapse, or where the bridge and tip need strong support. It is taken through a small chest incision, which leaves a scar and adds a recovery site.

Why grafts are needed

Nasal shape and airway depend on a bone and cartilage framework that can be weakened by injury, previous surgery or congenital weakness. Grafts support the tip, raise or straighten the bridge, keep the middle airway open and strengthen the nostrils.

Cartilage sources

  • Septal cartilage: from the same area, no extra incision; usually first choice, but may be insufficient after previous surgery.
  • Ear cartilage: curved and soft; good for tip and nostril support, limited for strong structural support.
  • Rib cartilage: plentiful and strong; for bridge reconstruction and major support loss, with a chest scar.

When is rib cartilage considered?

Multiple previous operations, saddle nose, major post-traumatic loss, a short or unsupported tip and some ethnic noses needing strong structure. It is less often needed in primary rhinoplasty.

Harvest and scar

A few-centimetre incision on the chest, in women often near the breast crease, is used. Pain and tenderness occur for some days. Rare specific risks related to the lining of the lung are explained beforehand.

Recovery

Nasal recovery is similar to other rhinoplasty but surgery may be longer and swelling slower. Rib cartilage may warp slightly over time; techniques aim to minimise this.

Frequently asked questions

Is rib cartilage needed in every rhinoplasty?

No; septal cartilage is usually enough in primary surgery.

Will there be a chest scar?

Yes, a few centimetres, usually in a concealable location.

Is it painful?

The donor site is tender for some days; pain is controlled with medication.

Can ear cartilage be used instead?

For smaller needs yes; it may be insufficient for major structural support.

Does rib cartilage dissolve?

Your own rib cartilage is generally permanent, though mild warping can occur.
