Phalloplasty builds a new penis from a flap of skin, fat, vessels and nerves, usually from the forearm or thigh. A urethra is often created within the flap. It is staged, and a penile prosthesis can later provide rigidity. It requires microsurgery and teamwork with urology.
Indications
Traumatic loss, penile cancer surgery, micropenis and female-to-male gender affirmation, after thorough medical, urological and, where needed, psychiatric assessment.
Techniques
- Radial forearm free flap: thin, pliable tissue connected by microsurgery; leaves a grafted forearm scar.
- Anterolateral thigh flap: pedicled transfer; may be bulky.
- Urethral reconstruction for standing urination.
- Penile prosthesis after healing.
Risks
Flap circulation problems, urethral stricture or fistula, infection, donor-site scarring, prosthesis complications and further surgery.
