Blepharoplasty removes excess skin and fat from the upper eyelids and corrects bags of the lower eyelids. The upper incision is hidden in the lid crease; lower-lid incisions are under the lashes or inside the lid. Bruising usually fades within 1–2 weeks.
Upper and lower blepharoplasty
Ageing causes excess upper-lid skin and fat, sometimes narrowing the visual field; lower lids develop bags, hollows and laxity.
Ptosis is different
A drooping lid is not always excess skin; a weak levator muscle (ptosis) needs different treatment, and a low brow can mimic excess lid skin. Brow position, levator function and dry eye are assessed.
Recovery
Usually under local anaesthesia with sedation. Swelling, bruising and a gritty feeling settle with cold compresses and drops; stitches come out within a week.
Risks
Dry eye, temporary blurred vision, asymmetry, lower-lid pulling (ectropion), scarring and very rarely bleeding behind the eye. Mention dry eye, thyroid disease or previous eye surgery.
Frequently asked questions
Will there be a scar?
The upper scar sits in the natural crease; the inside-lid approach for the lower lid leaves no external scar.
How long does bruising last?
Usually 1–2 weeks.
Can it improve vision?
If excess skin narrows the visual field, the field may widen after surgery.
Can under-eye bags be treated without surgery?
Mild hollows can be camouflaged with filler; marked bags need surgery for a lasting result.
